We recommend that you fill out this order form prior to
contacting us so that you will have decided on all the parameters beforehand.
Provide the following information for each item in the order:

1. Provide the following payment information:
a. Mastercard or Visa credit card number, expiration date, name on the card, billing address, and daytime phone number.
b. If paying by check, we will tell you the amount due and you can mail the check to Collage Gallery (96953 Blackrock Rd., Yulee, Florida 32097). Shipment will be sent after check clears the bank.
2. Shipping
a. Provide shipping address if different from credit card address.
b. Every effort is made to ship within one week of the order. If you require faster shipping, please let us know when you place your order.
